Emergency Information, Insurance Verification & Participation Waiver
Participants and coaches will NOT be allowed to participate in any RC Competes event without submitting the following prior to the start of the event. Teams entering in more than one competition DO NOT need to fill out more than one form. Only one waiver is needed for the entire season.

The RC Competes competition does not carry an accident or health insurance policy on participants. A participant's parent or legal guardian is responsible for costs incurred as a result of an accident or injury. Participants at the competition may be involved in strenuous physical activity. RC Competes directors and officials will not be held responsible or liable for an accident or injury to a participant.

I will also allow any pictures/videos of my student taken at competition to be used for promotional and training purposes."
